HISD Athletic Insurance Boys 2025-26
Available at the reduced cost of $35.00(+ SchoolPay fee) per athlete, per school year! If your child/children are covered on another medical plan, you then have the option to either purchase the additional insurance, or waive HISD's Athletic Insurance fee by doing to following:
-Be the parent or legal guardian of the athlete.
-Upload proof of medical insurance on your child/children(medical card).
- Provide a valid goverment or state-issued phote Idenification or driver's license.
- Complete and provide insurance waiver if declining HISD insurance.
Note: Since original signatures are required, Insurance waivers cannot be faxed. ALL ATHLETES MUST BE INSURED TO PARTICIPATE IN ANY ATHLETIC ACTIVITY!
