-              First Name, Last Name (Maiden Name if applicable)

-              Contact Information: Phone number and e-mail address

-              Social Security Number or TEA ID#

-              The years enrolled in the Houston ISD ACP

Texas ACP Transfer          $20.00  

You may request a program transfer by e-mailing Monica Mercado ([email protected]) the EPP Candidate Transfer Form along with the following information:

-              First Name, Last Name (Maiden Name if applicable)

-              Contact Information: Phone number and e-mail address

-              Social Security Number or TEA ID#

-              The years enrolled in the Houston ISD ACP

Out-of State ACP Verification       $25.00  

You may request out-of state enrollment verification or a program completion/certification status by e-mailing the appropriate documentation from your state to Monica Mercado ([email protected]) along with the following information:

-              First Name, Last Name (Maiden Name if applicable)

-              Contact Information: Phone number and e-mail address

-              Social Security Number or TEA ID Number

-              The years enrolled in the Houston ISD ACP
