I agree to attend the Midwest Clinic Trip in Chicago, Illinois  on December 15-18, 2021. I also agree to make payment by the specified date. Failure to meet payment deadlines could result in forfeiture of trip reservation. Total cost of the trip will be $300, based on four students per room. $300 is non-refundable. Eligibility is a major concern in dealing with the Midwest Clinic Trip. If a student is ineligible during the time period of our trip, he/she cannot attend the trip and this could result in a forfeiture of funds. Please pay online at: www.allenorchestra.org/store.
